For those that got certified more than 2 years ago, you need to complete this medical questionaire and mail it in. No Medical doctor is needed to certify it if its for a non-commercial class A

This site should be full of us that went out and got that license upgrade about that time. Just did my, took about three minutes to complete and stay legal. DMV sent me a notice in the mail (I had forgotten all about it).
